To our valued patients, please be advised that Myhealth Delta Family Doctors will be closing permanently with the last day of trading on Friday 4th August 2023.
We made every effort to stay open and continue delivering quality care to our beloved community but unfortunately we have simply been unable to continue doing so.
If you need to see a GP, please contact our sister clinics at Myhealth Forest Lake, 241 Forest Lake Blvd Forest Lake (Ph: 07 3278 9400).
If you would like your medical records to be sent to your new treating GP, please sign a request form at your new practice & the new practice can email this request to: deltafamilydoctors.reception@myhealth-stage.tblabs.site so that your new treating GP can continue your care.

We are a family General Practice located just steps away from the Discount Pharmacy at Kruger Shopping Centre. We offer all services in the field of general practice, including chronic health conditions, family medicine and with bulk billing available. We have full-time female and male doctors in our team who provide a comprehensive range of services to ensure that you and your family are given high-quality advice and health care.
Please also note our GPs do not provide JP services.

DNA/ CANCELLATION POLICY
As one of the few remaining bulk billing clinics in the area, we often have a large waiting list of people wishing to see our GPs. If you miss your appointment at short notice, this often will not allow us time to get in touch and assist other patients in need.
We therefore have a strict cancellation policy and by booking an appointment you are agreeing to this policy. If you do not attend your booked appointment, it will be recorded as a DNA (did not attend) and a $50 DNA fee will apply.
If reception is not notified of an appointment that needs to be cancelled or rescheduled by the close of business on the last working business day before your booked appointment, it will be classed as a late cancellation and a $50 late cancellation fee will apply.
For both DNA or Late Cancellation appointments, a $50 fee will be charged and you will not be able to rebook any further appointments until the fee is paid. Missed/late cancelled appointments will be marked as a ‘did not attend’ appointment and will stay on your file for a period of 12 months. Please see reception for full attendance policy details.
BULK BILLED SKIN CHECKS
We have fully bulk billed skin checks available in the practice with our skin doctor on Wednesdays. To book please call reception.
BOTOX INJECTIONS
Dr Golnar Mortezaei performs botox injections at a cost of $14 per unit. Contact reception for more information or to book an appointment.
