PRIVATE MIXED BILLINGS
(Most consultations are bulk billed, but private fees are applicable in some situations)
FEE PAYABLE=(1)CONSULT TIME FEE + (2)ANY ADDITIONAL FEES/SURCHARGES

(1)CONSULTATION TIME FEE
≤10Minutes. Bulk billed
(Unless additional fee/surcharge applicable)
>10mins calculated@$50/10mins($300/hour)
Medicare generally reimburses more than half of this fee. The proportion reimbursed by Medicare is generally less the longer the consult is.

(2)ADDITIONAL FEES/SURCHARGE
Saturday surcharge, 2pm onwards- additional $30
($83.65 fee with $53.65 Medicare rebate for standard 10min consult)
– Appointment non attendance, or late cancellation $30
– Late payment $30 (Payments are expected to be made on the day of consultation. If there is delay of over 1 month, late fee is applicable)
– Important recall administrative time $30
– Urgent/Same day fit in surcharge, $50
– Procedures to be quoted. Additional $100minimum
– Paperwork $50/10mins+GST=$300/hour+GST.

Unlike consultation fees, paperwork fees are not medicare reimbursable. This rate is based on the NSW government state insurance regulatory authority WCO002 $51/10minutes+GST

FULLY BULK BILLED FOR ANY OF THESE SITUATIONS
– Non urgent single routine issue, weekday or Saturday before 2pm, consult ≤10mins. (For families if average consult duration is ≤10minutes per patient, all will be bulk billed even if one consult is slightly over 10 minutes)
– weekday presentation only for vaccination or wound dressing done by nurse.
– Regular patient consult time over 10 minutes, but Medicare eligible for care plan, GPMHTP, or age > 75health assessment.

PRIVATE MIXED BILLING CONCEPT
– Patients will be bulk billed for non urgent routine/simple single problems presenting on weekdays resolved within 10 minutes or tasks performed principally by the nurse.
– If there is a complex problem, a couple of presenting issues, or time consuming paperwork, patients will pay a fee that covers extra GP consultation time taken. Paying a small fee to adequately fund additional time avoids problematic rushing, reduces the risk of errors occurring and gives time for better communication with patient. We also do ask for an additional fee contribution if we are giving up some of our own family time to serve your needs on a Saturday afternoon, or sacrificing our break times to fit you in.
– Patients are expected to book a follow up appointment via telehealth or face to face to follow up on all tests done. A dedicated results appointment is bulk billed unless unexpected complex time consuming issues arise taking over 10 minutes to resolve. If a patient has not booked their own results appointment and there has been significant administrative burden to recall patients including multiple direct phone call attempts where patients do not answer, fees are applicable to cover for that time and administration. Patients choosing to defer review of results until a new issue presents will result in potentially problematic delayed management and a longer consultation that will attract additional fees.
– Telehealth consults are generally appropriate for simple results follow up, but can be problematic for other concerns. Telehealth consults for lengthy issues will incur major out of pocket expense due to capped limited Medicare rebate. The lack of examination makes diagnosis problematic.
– This mixed billing policy uniformly applies to all patients irrespective of concession status, age or any other factor. Regular patients known to doctor are more typically bulk billed due to time efficiency in doctor knowing their past history. There may be ability to utilise special Medicare item numbers such as care plans which adequately funds additional time, only available because Medicare permits it for regular patients of that GP having certain conditions.
– While most patient presentations in this mixed billing model are bulk billed, to avoid disappointment, we need patients to have the mindset of valuing the GP sufficiently to expect paying a fee.
– This medical centre has mixed billing private clinicians as opposed to a fully bulk billing public hospital clinic. Within this private mixed billing medical centre, variations exists between practitioners in the application of mixed billings. If follow up is required by medical practitioner that is not the patient’s usual practitioner either by patient choice or necessity due to duty of care or in urgent settings, the patient will be expected to pay the fees of the practitioner involved in their care.
– In an emergency, it is advised to call 000 or go directly to Westmead hospital emergency department. This is in the interests of obtaining best urgent medical treatment, avoiding delay and unnecessary cost.
