BY USING OUR PLATFORM YOU ACCEPT THESE TERMS
We encourage you to read these Terms carefully. By accessing or using the Platform in any way, you acknowledge and confirm that you have read and understood these Terms and agree to abide by these Terms. If you do not agree to any of the Terms, you must not use the Platform.

If you are a parent or legal guardian allowing a minor to access and/or use the Platform, you agree to: (1) supervise the minor’s use of the Platform; (2) assume all risks associated with, and liabilities resulting from, the minor’s use of the Platform; (3) ensure that the content on the Platform is suitable for the minor; (4) ensure all information submitted to us by the minor is accurate and (5) provide the consents, representations and warranties contained in these Terms on the minor’s behalf.
NEW PATIENT REGISTRATION FORM
When you sign up to a selected Myhealth practice as a new patient, you may complete our New Patient Registration Form (Registration Form).
Any information that you provide in the Registration Form will be used and disclosed in accordance with our Privacy Policy.
Our Registration Form uses the Google Maps API(s) for address verification. By using the Registration Form, you agree that you are bound by Google’s Terms of Service
HOW YOU MAY USE MATERIAL ON OUR PLATFORM
Unless otherwise indicated, we are the owner or the licensee of all intellectual property rights on our Platform, and in the material published on it. Those works are protected by copyright laws and treaties around the world. All such rights are reserved.
You must not, without the prior written consent of ourselves or except as expressly permitted by these Terms: (1) copy or use, in whole or in part, any content; (2) reproduce, reverse engineer, retransmit, distribute, disseminate, sell, publish, broadcast or circulate any content to any third party; or (3) breach any intellectual property rights connected with our Platform, including by altering or modifying any of the content, causing any of the content to be framed or embedded in another website, or creating derivative works from the content.
Subject to your compliance with these Terms, we grant you a personal, non-exclusive, royalty-free, revocable, worldwide, non-transferable licence to download and use our Platform on your devices and access and view any content, in accordance with these Terms. All other uses are prohibited without our prior written consent.
